Pediatric Cervical Spine Injuries: Evaluation And Management

It is an injury with deleterious emotional, physical, and financial implications. The incidence of spinal cord injury is not well documented in the literature particularly for children. However, it is known that the mechanism of injury, age distribution, and outcome vary significantly between developed and developing countries. In developed countries the incidence is 15-40 per million children per year. Motor vehicle crashes are the leading cause of injury and the majority of these children have an associated head trauma. When injury occurs, the cervical spine is the most common site of injury in 75% of children under 8 years of age. In older children and adolescents the pattern of injury is similar to that seen in adults. Children susceptibility to spine injury is related to the anatomical and biomechanical differences between their immature spine and the mature adult spine. As a result of these differences, 60 to 80% of all pediatric spine injuries occur in the cervical region. A missed or delayed diagnosis of spinal cord injury in the emergency department can lead to disastrous outcome. Therefore, in children with trauma, expeditious cervical-spine clearance is important and requires knowledge of the anatomical and biomechanical difference. A child with suspected spinal cord injury should be evaluated in an organized approach by a multidisciplinary trauma team to optimize outcome.Keywords: Pediatric, spine, injuryTurkiye Klinikleri J Surg Med Sci 2007, 3(50):40-46
