Aortic Traumas

Injury of the intrathoracic great vascular structures occurs in three varying mechanisms which are shearing ,compression and intraluminal hiperextension. Most of the traumatic injuries against thorax and aorta are due to penetration of bullet or stab wounds aortic rupture is the most accounted visseral patology with blunt trauma of thorax. Lesions due to trauma of the aorta are simple contision intramural hematoma, rupture leading to dissection or pseudoaneurism. Rate of aortic trauma is approximitaly %1-2 when public vascular injuries are evaluated. Traumatic aortic rupture is mostly occurs in isthmic portion of the aorta. While %25 of patients have retrosternal pain, %25 is asemptomatic .On chest XR, mediastinal widening more than 8 cm is established in %93 of patient. In traumatic thoracic aortic rupture, there are mainly three technics used, which are aortic clampage, distal perfusion and cardiopulmonary bypass.
