Intoxication With Tricyclic Antidepressants And Therapeutic Approach

The main symptoms of depression are feelings of intense sadness and despair, mental slowing and loss of concentration, pessimism, lack of pleasure and interest, self-deprecation and variable agitation. Physical changes may also occur in severe forms of depression. Patients who has major depression display suicidal behaviour during their lifetime. Suicidal attempts with tricyclic antidepressant drugs that are ending with acute intoxication have a great pharmacological importance. Acute poisoning with a tricyclic antidepressant often is clinically complex. Symtoms are due to anticholinergic effects and actions on the heart. A tricyclic antidepressant-intoxicated patient must be treated immediately in an intensive care unit. Cardiac monitorization is essential because of cardiotoxicity potential of these agents. Activated charcoal for prevention of absorption, sodium bicarbonate for correction of acidosis, volume replacement and sympathomimetic agents for hypotension, lidocaine and bretylium for cardiac arrhythmias is often recommended along with supportive care.Keywords: Tricyclic antidepressants, suicidal attempts, acute intoxication, anticholinergic effectsTurkiye Klinikleri J Surg Med Sci 2006, 2(46):107-111
