Midtrimester Bleeding

It has the same causes as bleeding that occurs during the third trimester. It may originate from the vulva, the vagina and the vaginal portion of the cervix, from a low lying placenta or placenta previa and from marginal and (rarely) total placental separation.The most common significant reasons for bleeding are placenta previa and abruptio placenta; however, the differential diagnosis must include cervical pathology such as erosion, polyp, vasa previa and the early stages of cervical insufficiency or rarely cancerous lesions. Genital infections, trauma or systemic diseases can result in bleeding any time throughout pregnancy. However, more than 50% of episodes are of unknown origin. Any bleeding, however slight, with or without pain or uterine contractions, must be investigated.
Current management of these pregnancies should involve a careful physicial examination and ultrasound to evaluate the origin of bleeding. Most serious problems that occur during mid pregnancy involve a risk of very early delivery or fetal loss, particularly in patients with repeated previous midpregnancy loss, cervical insufficiency and repeated early bleeding from the placenta implantation site.Keywords: Vaginal bleeding, plasental disorders, cervical insuficiency, pregnencyTurkiye Klinikleri J Surg Med Sci 2006, 2(24):13-18
