Pediatric Oncologic Emergencies
Dr. Vural KESİK,a Dr. Vedat KÖSEOĞLUa
aPediatrik Onkoloji BD, GATA, ANKARA Advances in neoplastic treatment modalities for children have greatly increased the cancer survival rate. Unfortunately, at the current time, this therapy often involves complications that require intensive care. According to the most recent statistics, 77% of all children diagnosed with cancer become 5-year survivors. This figure has increased by about 40% over the past 20 years. Despite these figures, cancer continues to be the leading cause of death by disease in children with an estimated 1500 deaths per year. Emergencies in patients with cancer may be directly related to the underlying cancer or may result from treatment. These emergencies can be classified into three types: A space-occupying lesion causing pressure or obstruction, a tumor creating metabolic or hormonal problems (paraneoplastic syndromes), or complications arising from treatment such as tumor lysis syndrome or septic shock with respiratory or cardiac insufficiency. More recent literature suggests that earlier identification and treatment of problems results in better outcome. The purpose of this article is to discuss oncological life threatening events, including assessment parameters, diagnostics, pathophysiology, and management criteria. Keywords: Oncologic emergencies, childrenTurkiye Klinikleri J Surg Med Sci 2006, 2(33):94-112
