Mechanical Neurologic Emergencies In Hematology

aNöroşirürji AD, İstanbul Üniversitesi Cerrahpaşa Tıp Fakültesi, İSTANBUL Patients with hematologic diseases may present with different pathologies, resulting in intracranial hypertension or spinal cord compression. Recognition and evaluation of these symptoms and signs before neurologic deterioration is as important as the treatment of these cases in emergency circumstances . Intracranial hypertension may begin with headache and if not treated appropriately it may progress to cerebral herniation syndromes when the compensatory mechanisms are exhausted after loss of compliance. A cranial tomography must be performed in all patients with the suspect of intracranial hypertension. These patients are treated according to the pathology which is leading to intracranial hypertension. In those patients, if a surgical intervention is needed, each patient must be evaluated individually regarding the primary disease. Spinal cord compression is a common entity in the daily practice of hematology, because multipl myeloma has the highest incidence of spinal cord compression compared to other tumors. In patients with spinal cord compression prompt diagnosis and treatment with either radiotherapy or surgery is essential. The management goal for patients with metastatic spinal disease includes the restoration or preservation of neurologic function, management of stability and relief of pain .
